Elder Serve will send a registered nurse to your home to evaluate your needs and determine which services will best help you.
Step 3-
Elder Serve will coordinate with your personal physician. With your doctor's approval, we will coordinate and begin your services

ElderServe was so wonderful to my mother when she needed care at home. They not only sent in a whole range of talented health care professionals (physical therapist, nutritionist, nurse, aide, occupational therapist, social worker), but they spent the time to teach her about managing her hypertention and diabetes. They even arranged for her to go to a wonderful adult day program two days a week, so she could get out and socialize in a safe setting. I was able to keep my promise to her, to never put her in a nursing home, but I know that she's in good hands. I don't know what I would have done without them.
